Edelweiss Aesthetics PLLC
Patients’ Rights Notice – Your Cost of Care
At Edelweiss Aesthetics PLLC, we believe in transparent and upfront pricing.
- We do not accept or bill health insurance. All services are self-pay and due at the time of treatment.
- The current cost of our services is publicly listed on our website at: edelclinic.com
Your Rights Under the Federal No Surprises Act
Because our prices are listed on our website, your Good Faith Estimate will typically reflect those published rates. If a treatment plan changes or additional services are recommended, we will update your estimate accordingly.
Should we be unable to quote a specific amount due the inability to predict in advance your specific treatment needs or diagnosis, we will disclose an estimate of the out-of-pocket costs you will be required to pay.
As a self-pay practice, you have the right to receive a Good Faith Estimate explaining how much your medical care will cost before you receive any non-emergency services.
Timing of Estimates:
- If you schedule your appointment 10 or more business days in advance, you will receive your Good Faith Estimate within 3 business days of scheduling
- If you schedule at least 3 business days in advance, you will receive your Good Faith Estimate within 1 business day of scheduling
- You may request a Good Faith Estimate at any time, and we will provide it within 3 business days of your request
- If your treatment plan changes or additional services are recommended, we will update your estimate accordingly
Disputing a Bill
If you receive a bill that is $400 or more above the Good Faith Estimate, you have the right to dispute the charges. For more information, visit:
www.cms.gov/nosurprises/consumers or call 1-800-985-3059.
Massachusetts Patients’ First Law
As a Massachusetts provider, we also follow pricing transparency laws. This means you have the right to request a written disclosure of the estimated charge for any service before it is provided. We will provide this information within 2 business days of your request.
If you are referred to another provider for further services, you should verify with that provider whether they accept your insurance and what out-of-pocket costs may be.If you believe you did not receive adequate advance notice of your out-of-pocket costs, you may learn more or file a complaint at: Massachusetts Patients’ First – Pricing Transparency.
